Community Transportation is a service available to Medicaid eligible consumers when transportation cannot be provided through another Medicaid transportation service or community resource.

Community Transportation provides transportation to and from Medicaid coverable services. It is not to be used to take you any place that is not a Medicaid coverable service.

The Guernsey County Department of Job and Family Services (GCDJFS) encourages each consumer to choose the mode(s) of transportation that addresses their medical condition and timeliness concerns.

Community Transportation may be available through these modes of service:

• Guernsey County Senior Citizen Center is under contract with the GCDJFS as a choice for senior citizens to receive transportation services to and from their home and a Medicaid coverable service.  Transportation is limited to Guernsey County and New Concord for seniors 60 years or older. • South East Area Transit is under contract with the GCDJFS as a choice for transportation services to and from the consumer’s home and a Medicaid coverable service.  To schedule a ride, call in advance of your appointment at 740-439-3393

· If you fail to cancel a scheduled ride, you may not be eligible for additional transportation service. This suspension from service may last up to 6 months, depending upon your past failures to cancel.
